Your employer gives you Form W-2 at the end of the calendar year to show the total amount of tax they withheld from your paychecks. Your employer also provides a copy of the Form W-2 to the IRS, Social Security Administration and some state taxing authorities. These taxing entities match up the amounts you claim as income with the amounts your employer reports they paid you. Because your employer sends this form to the IRS, you do not need to file it with your tax return.

New Postal Regulations about Wafer Seal Labels
The US Postal Service now requires that all wafer seal tabs used to seal booklets should not have any perforations. in general, all booklets need 3 tabs of 1-1/2" to close from all sides (1 per side). For larger or very heavy booklets, the USPS recommends 2" wafer seals. Booklets that do not comply with the new standards will not be eligible for machinable or automation letter prices. they will pay regular retail postage but the stuff will be delivered. the rules does not effect persinal users paying standard postage.

After processing, the said accomplished document is sent back to the testing facility for storage if so required by law. This would be the case for all federal employees where the law dictates that the samples be kept for a period of 1 (one) year from the conduction of the testing to give time for questioning and the filing of complaints in case the said results are refuted. Once a complaint is filed, the sample is then pulled out of storage for re-testing and again the document is updated for reference purposes.
As seen above, the said chain of custody form is not a form by itself but a process of accountability which leaves a historical trail of the processing a test specimen has been through that could provide essential information with regards to validity and legal issues. The Mandatory Guidelines for Federal Workplace Drug Testing Programs was intended for within the Federal Government but has been used as the pattern for subsequent application in the private sector which may or may not follow all the statutes of the said guidelines.
As a result of the need for chain of custody monitoring even FDA approved portable test kits that are used by private companies have provisions to strictly follow the stated laws. Urine samples are collected and documented for testing off-site in an accredited laboratory. The sample is sealed and placed inside either a box or plastic container that is again sealed for security prior to transport. The courier is also required to document the transfer of the sample or samples from source (requesting company) to the testing facility that will conduct the necessary tests requested. The samples can then either be tested immediately or stored in conditions stated within the collection guidelines till such time that the test can be conducted. Independent laboratories that are accredited to provide testing services are required by law to keep test and employee information for a minimum of two years after which destruction of the said records is allowed upon verification that none of them are subject to legal cases.
All of the processes involving the chain of custody document serves as assurance to the test subject that the specimen that was provided was handled and tested in the enumerated procedures of the federal Drug-free Workplace program. All information including the chain of custody form are considered to be highly private and confidential for both federal and private employees who undergo these tests. Even the results that are given by the MRO that are documented and reported are considered confidential as part of the stated regulatory guidelines.
Major differences in federal and Non-Federal Chain of Custody forms is that Federal CCF forms have 5 copies; Copy 1 is to be furnished for the Laboratory; Copy 2 for the MRO (Medical Review Officer); Copy 3 is for the Collector; Copy 4 is for the Employer and the last one for the Donor or test subject. This is a reflection of the full accountability principle defined by the revised Mandatory Federal Workplace Drug Testing Program. what is NCR paper?
NCR means "Non Carbon Required" if you remember years ago, a multi copy form came with regular paper with some black or blue carbon sheets between each part, the carbon sheets had made the image able to go true for the next part, Carbon-less paper, commonly known as NCR paper, was first produced by the NCR Corporation. For three copies, there would be a white top part and two colored parts below, called three-part or triplicate. Four copies would be called four-part or quadruplicate.

For most every type of shipping there is a unique Bill of Lading layout that provides specific instructions for the shipping carrier. Before discussing the 12 common types of Bill of Lading forms.
12 Common Types of Bill of Lading Forms & When To Use Them
1. Straight Bill of Lading: is typically used when you shipping merchandise to a customer. The "Straight Bill of Lading" is for shipping items that have already been paid for.
2. To Order Bill of Lading: Used for shipments when payment is not made in advance. This can be shipping to one of your distributors or a customer on terms.
3. Clean Bill of Lading: A Clean Bill of Lading is simply a BOL that the shipping carrier has to sign off on saying that when the packages were loaded they were in good condition. If the packages are damaged or the cargo is marred in some way (rusted metal, stained paper, etc.), they will need issue a "Soiled Bill of Landing" or a "Foul Bill of Landing."
4. Inland Bill of Lading: This allows the shipping carrier to ship cargo, by road or rail, across domestic land, but not over seas.
5. Ocean Bill of Lading: Ocean Bills of Lading allows the shipper to transport the cargo over seas, nationally or internationally.
6. Through Bill of Lading: Through Bills of Lading are a little more complex than most BOLs. It allows for the shipping carrier to pass the cargo through several different modes of transportation and/or several different distribution centers. This Bill of Landing needs to include an Inland Bill of Landing and/or an Ocean Bill of Landing depending on its final destination.
7. Multimodal/Combined Transport Bill of Lading: This is a type of Through Bill of Lading that involves a minimum of two different modes of transport, land or ocean. The modes of transportation can be anything from freight boat to air.
8. Direct Bill of Lading: Use a Direct Bill of Lading when you know the same vessel that picked up the cargo will deliver it to its final destination.
9. Stale Bill of Lading: Occasionally in cases of short-over-seas cargo transportation, the cargo arrives to port before the Bill of Landing. When that happens, the Bill of Landing is then "stale."
10. Shipped On Board Bill of Lading: A Shipped On Board Bill of Lading is issued when the cargo arrives at the port in good, expected condition from the shipping carrier and is then loaded onto the cargo ship for transport over seas.
11. Received Bill of Lading: It is simply a Bill of Lading stating that the cargo has arrived at the port and is cleared to be loaded on the ship, but has not necessary mean it has been loaded. Used as a temporary BOL when a ship is late and will be replaced by a Shipped On Board Bill of Lading when the ship arrives and the cargo is loaded.
12. Claused Bill of Lading: If the cargo is damaged or there are missing quantities, a Claused Bill of Landing is issued.

Do you remember being at the diner, as the waiter wrote down your order on those thin paper notepads with the blue carbon paper sheet inside of them?
If you notice, you don’t see them around too much anymore. Why? Because as the acronym implies, NCR (No Carbon Required)
Today the paper you write on itself has the mechanisms for transferring what you wrote on the top page to the sheets below without the need of the old famous carbon sheet paper.
NCR paper was invented by two chemists working for the NCR Corporation as a substitute for carbon paper.
The back of NCR paper is coated with micro-encapsulated dye and the front is coated with a reactive clay.
When the NCR paper is written on with a pen or other writing (pressure) tool, the pressure from the point of the pen causes the micro-capsules coated on the back to burst spilling the dye onto the front of the sheet below making contact with the reactive dye and therefore creating a mark.
NCR paper is available in many different color's and what coating each sheet gets depends on the order and number of parts in the Carbon-less Form.
For example, in a 3 part (Carbon-less) NCR form:
White CB (coated back): The 1st sheet is usually white in color and does not require a clay coating on the front since it will be written on directly. The back is coated with the micro-encapsulated dye in order to transfer to the second sheet.
Yellow CFB (coated front and back): The 2nd NCR sheet is usually yellow which will be coated with clay on the front to react to the dye from the back of part 1 and also coated with the dye on the back in order to transfer the writing to part three.
Pink CF (coated front): Sheet 3 will be pink and coated on the front with the clay coating only since it has nothing more to transfer to through the back.
Note: Any of the above papers can also be printed on the back without preventing the coating to do its job. (normally used for Terms and Conditions)
NCR Form Binding Carbon-less forms are used by a wide range of businesses and institutions creating the need for different types of binding for specific uses. Here are a few examples of common NCR form binding types:
Edge Glued: Standard and least expensive when you order a small quantity, when you order more than 5,000 sets, Snap Set Carbonless Forms will be about the same in price.
Snap Set: A step up from the standard edge glued form, a snap set can keep parts together and allows for parts to be easily “snapped” off via a micro perforation at the stub.

Security paper is a specific type of paper that uses security features to identify documents as original. Security paper may also come with specail features that can prevent fraud by using tamper resistant options. These types of papers are used by the financial industries and are also used to identify documents such as checks, passports, licesnse, birth certificates, monetary currency and academic certificates.
Security paper has now become more advanced using a lot of different features such as hidden watermarks and pantographsto hinder duplication and tampering. These features cannot be duplicated on copying equipment and informs bank tellers or notaries that the document presented to them is the original document. Security paper is also acid free which allows it to be preserved for a longer amount of time than regular paper.
To validate an original watermarked document, simply rub the translucent watermarked area with a coin. When this watermark is rubbed it will darken validating the authenticity of the document.
Often times solvents, bleach, water or other chemicals are used to alter what is printed on a document, but with use of Security Paper spots will appear indicating that the document has been tampered with. Another way of altering a document is to erase what has been printed. When someone attempts to erase something that has been printed or written on the security paper it will destroy the paper fibers.
One of the options of security papers is a use of UV Fibers. UV Fibers have been manufactured into the paper stock which can often times be seen with the naked eye and will brighten or glow when viewed under an ultraviolet lamp, Another option of security papers is UV dull paper, When a paper stock is UV dull it means that it will not reflect ultraviolet light. This means that the paper is suitable for ultraviolet inks, which can be used to show a hidden symbol, control number or other variable data, which can then be seen under ultraviolet lamps.
