Integrated Card Form
this is a 8-1/2 x 11" form with a 3-3/8 x 2-1/8" Integrated card, the card is die cut into the same stock as the form, centered on the bottom, the card have a 2 mil back lamination, no lamination on the front, the back liner will remain on the form after you pull of the card.
the card is being used for
Membership Cards, Club Cards, Security Passes, Business Cards, ID Cards, Insurance Cards, Health Care, Marketing applications etc.
